– Hannah is a Board-Certified Family Nurse Practitioner with years of experience in both conventional and Functional Medicine. She is dedicated to helping patients take control of their health, find the root causes of chronic conditions, and achieve true healing through integrative approaches.
Victoria Ward, PA
– Victoria is a Board-Certified Physician Assistant with expertise in pain management, internal medicine, and integrative care. She believes in a patient-centered, team-based approach to wellness and uses her knowledge of functional medicine to empower her patients to improve their quality of life through sustainable lifestyle changes.

Examples of support offered through CPCF include: • Assistance with the cost of medications • Assistance with the cost of office visits • Help with purchasing medical equipment and supplies • Coverage for primary care and specialist appointments Our Mission: We aim to remove barriers to high-quality health care by providing patient-centered support and treating every individual with excellence in care, service, and access.
