MedNOW Now Offers an In-House CCM Team
At MedNOW, our mission is to provide comprehensive care that supports your health and well-being. That’s why we’re excited to share information about our Chronic Care Management (CCM) Team, designed specifically to assist Medicare patients living with chronic conditions.
What is CCM?
CCM is a Medicare program designed to improve chronic care management by assisting with the coordination of services, improving the management of chronic health concerns, reducing hospital visits, and improving patients’ quality of life.
How MedNOW’s CCM Works
To ensure the highest quality of care, MedNOW has brought CCM in-house and established a dedicated Value-Based Care Team, including two CCM coordinators. Here’s what our team will do for you:
• Conduct monthly check-ins to monitor your health.
• Coordinate follow-up visits, prescription refills, medical equipment needs, and specialist appointments.
• Provide immediate updates to your provider after each CCM visit to ensure seamless care.
• Offer health education, referrals, and support to help manage your chronic conditions.
Examples of Chronic Conditions Eligible for CCM
The program is available to Medicare patients with conditions such as:
• Diabetes
• Hypertension
• Arthritis
• Cardiovascular disease
• Chronic Obstructive Pulmonary Disease (COPD)
• Depression
• Cancer
• Alzheimer’s disease and more

That’s why we want to remind you about our Urgent Care Services available at two of our locations —with a third location coming soon! Our Urgent Care Services are designed to treat non-life-threatening conditions quickly and conveniently, so you can avoid unnecessary ER visits and long wait times. Whether it’s minor injuries, illnesses, infections, or other urgent concerns, our experienced providers are here to help. Current Urgent Care Locations: Aurora - 15101 E. Extended Hours: Monday – Friday 7am to 6pm Saturday 8am to 4pm Sunday 8am to 12pm We are so excited to be able to offer Urgent Care Services at our Aurora and Colorado Springs locations. In the near future, our Sheridan Blvd location will also be offering Urgent Care services as well. We will make sure to post an update once our services are available!
