MedNOW Now Offers an In-House CCM Team

At MedNOW, our mission is to provide comprehensive care that supports your health and well-being. That’s why we’re excited to share information about our Chronic Care Management (CCM) Team, designed specifically to assist Medicare patients living with chronic conditions.

What is CCM?
CCM is a Medicare program designed to improve chronic care management by assisting with the coordination of services, improving the management of chronic health concerns, reducing hospital visits, and improving patients’ quality of life.

How MedNOW’s CCM Works 
To ensure the highest quality of care, MedNOW has brought CCM in-house and established a dedicated Value-Based Care Team, including two CCM coordinators. Here’s what our team will do for you:
• Conduct monthly check-ins to monitor your health.
• Coordinate follow-up visits, prescription refills, medical equipment needs, and specialist appointments.
• Provide immediate updates to your provider after each CCM visit to ensure seamless care.
• Offer health education, referrals, and support to help manage your chronic conditions.

Examples of Chronic Conditions Eligible for CCM
The program is available to Medicare patients with conditions such as:
• Diabetes
• Hypertension
• Arthritis
• Cardiovascular disease
• Chronic Obstructive Pulmonary Disease (COPD)
• Depression
• Cancer
• Alzheimer’s disease and more

The CPCF is dedicated to improving access to healthcare by assisting individuals with costs related to both medical needs. Many patients in our community face barriers such as high medication costs, lack of insurance, and the inability to afford high co-payments and durable medical equipment like walkers, wheelchairs, and oxygen tanks. These obstacles can significantly affect a person's ability to maintain their health and quality of life.
