MedNOW Now Offers an In-House CCM Team

At MedNOW, our mission is to provide comprehensive care that supports your health and well-being. That’s why we’re excited to share information about our Chronic Care Management (CCM) Team, designed specifically to assist Medicare patients living with chronic conditions.

What is CCM?
CCM is a Medicare program designed to improve chronic care management by assisting with the coordination of services, improving the management of chronic health concerns, reducing hospital visits, and improving patients’ quality of life.

How MedNOW’s CCM Works 
To ensure the highest quality of care, MedNOW has brought CCM in-house and established a dedicated Value-Based Care Team, including two CCM coordinators. Here’s what our team will do for you:
• Conduct monthly check-ins to monitor your health.
• Coordinate follow-up visits, prescription refills, medical equipment needs, and specialist appointments.
• Provide immediate updates to your provider after each CCM visit to ensure seamless care.
• Offer health education, referrals, and support to help manage your chronic conditions.

Examples of Chronic Conditions Eligible for CCM
The program is available to Medicare patients with conditions such as:
• Diabetes
• Hypertension
• Arthritis
• Cardiovascular disease
• Chronic Obstructive Pulmonary Disease (COPD)
• Depression
• Cancer
• Alzheimer’s disease and more

In observance of Labor Day, all MedNOW Clinic locations will be closed on Monday, September 1, 2025. We will reopen with normal business hours on Tuesday, September 2, 2025. We encourage patients to plan ahead for any prescription refills, routine appointments, or medical needs prior to the holiday. We are excited to introduce Dr. Sean Kark, DO, who has recently joined the MedNOW Clinics team as a Family Physician.
