If you are a new patient, we ask that you arrive at your appointment 30 minutes early to fill out the necessary paperwork. To reduce the time needed prior to your appointment, you can download the New Patient Packet by clicking the button below.

Are you meeting with your provider through our Telehealth platform? Please fill out the Telehealth Consent Form prior to your visit and email it to info@mednowclinics.com!

Please complete the waiver below if you have made an appointment for a minor. You can either bring this form with you when you come in for your appointment, or you can email the completed form to info@mednowclinics.com
